Comments:
A renewal inspection was initiated and concluded with the provider on 12/13/2021. There were 11 children present, ranging in ages from 16 months to 5 years, with 1 staff supervising until 1:40 pm when the second staff arrived. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records 1 household member record and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-320-I
Description: Based on observation and interviews it was determined that the provider failed to ensure the absence of fire hazards. Evidence: Wires were observed coming out of the ceiling in the basement where a smoke detector is supposed to be installed. It was reported that the smoke detector was removed due to the basement being painted.

Plan of Correction: The smoke detector will be reinstalled.

Standard #: 8VAC20-800-570-A
Description: Based on observation and interviews it was determined that the provider did not always maintain a point system not to exceed 16 points. Evidence: Upon arrival the licensing inspector (LI) observed the provider caring for 11 children ages, 16 months, 2 years, 2 years, 2 years, 2 years, 2 years, 3 years, 4 years, 4 years, 4 years, and 5 years for a total of 22 points from approximately 1:00 pm - 1:40 pm. It was reported that one assistant left the home at 1:00 pm while a second assistant reported to the family day home at 1:40 pm.

Plan of Correction: The provider will ensure that the appropriate amount of staff is onsite to not exceed 16 points per provider or caregiver.
