Technical Assistance:
A carbon monoxide detector was observed on site for this facility that is licensed to serve children 3 months through 12 years of age.

Comments:
A monitoring inspection (virtual component) was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

This inspection was initiated and concluded on 12/17/2021. The Provider was contacted and a virtual inspection was conducted. There were 10 children present (21 points), ranging from toddler to preschool age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 11 children records, and 3 caregiver records, and 2 household records were reviewed.

Information gathered during the inspection determined that there were a total of (4) non-compliances with the applicable licensing Family Day Home (FDH) standards. These violations were documented on the Violation Notice issued to the program.

Violations:
Standard #: 8VAC20-800-780-B
Description: Based on review, 2 of 11 children files indicated that annually, the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

Evidence -
1. On the date of inspection (12/17/2021), the following children did not have an updated review, signed and dated by the child's parent: Child #1 (signed, but no date listed), and Child #7 (most current review signed and dated - 05/26/2020).
2. The Provider confirmed this finding on the date of inspection.

Plan of Correction: Each parent will be contacted tonight to review, sign, and date.

Standard #: 8VAC20-800-800-B
Description: Based on observation, the provider did not document in writing each review of the emergency preparedness and response plan (EP&RP) at least on an annual basis.

Evidence -
1. The last review documented for the Family Day Home (FDH) EP&RP was dated - 11/20/2020.
2. The EP&RP was posted on the main bulletin board in the lower right hand corner.

Plan of Correction: The plan will be reviewed, signed, and dated today. No updates are needed.

Standard #: 8VAC20-800-100-A
Description: Based on review, 3 of 11 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to the child's attendance or within 30 days after the first day of attendance.

Evidence -
1. On the date of inspection (12/17/2021), documentation of a physical examination was not available for review for the following children: Child #1 (start date - 10/21/2019), Child #8 (start date - 03/29/2021), and Child #10 (start date - 01/31/2021).
2. Child #1, Child #8, and Child #10 were all in attendance on the date of inspection.

Plan of Correction: Each parent will be contacted to remind them to get this documentation for the FDH.

Standard #: 8VAC20-800-60-B
Description: Based on review, 2 of 11 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (12/17/2021), the following children had listed 1 of 2 emergency contacts: Child #4, and Child #7.
2. This was confirmed on the date of inspection by the Provider.

Plan of Correction: I will try again to obtain another emergency contact for each of these children.
