Comments:
This inspection was conducted by licensing staff using an in person component protocol, including telephone contacts, documents review, interviews.

An in person component monitoring inspection was conducted on 12/6/2021. There were no children present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of one child records and one staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on interviews and review of records the provider failed to conduct and document monthly evacuation procedures with children in care.
Evidence:
1. The provider stated that she has not conducted any evacuation (fire) drills.

Plan of Correction: The provider will ensure that fire drills are conducted monthly and documented.

Standard #: 22VAC40-111-830-B
Description: Based on observation and interviews the provider failed to practice shelter-in-place drills twice per year.
Evidence:
1. The provider stated that she has not conducted shelter-in-place drills this year.

Plan of Correction: The provider stated that she will ensure shelter-in-place drills are conducted two times a year.

Description: Based on observation and interviews the provider failed to keep a record of any emergency evacuation and shelter-in -place drills for one year.
Evidence:
1. The provider was not able to produce any documentation of emergency drills being conducted.

Plan of Correction: The provider will ensure that emergency drills are documented when conducted and the document will be maintained for a year.

Description: Based on observation and interviews the provider failed to have a current menu posted.
Evidence:
1. There was no menu posted.

Plan of Correction: The provider will ensure that a menu is posted.

Standard #: 22VAC40-111-230-A
Description: Based on interviews and observation the provider failed to have documentation of a current first aid and CPR certificate.
Evidence:
1. The provider stated that she could not find her first aid and CPR certification. She also stated that she will need to contact the trainer to see about a new class.

Plan of Correction: The provider will search her files and if unable to find a current first aid and CPR certificate she will register for another class.

Standard #: 22VAC40-111-240-A
Description: Based on observations the areas and furnishings of the home inside are not being maintained in a clean, safe and operable condition.
Evidence:
1. The room used for daycare had walking space approximately three feet wide. The two chairs in the room had books and clothes in them. The room had furniture, table and stackable washer and dryer piled up. Everything from the closet had been removed and placed in this room. No place for the children to play.
2. The kitchen table and counters were covered with old, empty food containers and fresh food. The kitchen floor was dirty.
3. The den area was cluttered with clothes, covers, open containers sitting around the room.

Plan of Correction: Mopped the floor, cleaned the kitchen and are getting rid of the clutter in the childrens room.

Standard #: 22VAC40-111-460-D
Description: Based on observation and interviews the provider was not able to produce a copy of recent rabies vaccination for the six dogs and three cats.
1. The provider has six dogs and three cats. The provider could not find the recent vaccinations for any of the animals.

Plan of Correction: The provider stated that she has an appointment today at 4:00 pm for four of the dogs to get rabies vaccination.

Standard #: 22VAC40-111-460-I
Description: Based on observation and interviews during the inspection the provider failed to keep the food and water dishes inaccessible to the children.
Evidence:
1. The food and water bowls are located in the kitchen next to the doorway going into the daycare room.

Plan of Correction: The provider will ensure that the animals food and water dishes are not accessible to the children.
