Technical Assistance:
The July 2, 2021 memo regarding the requirement for a carbon monoxide detector in the home was emailed to the provider.

Safe Sleep was discussed. Written information has been emailed.

Standard 210.A.3 was reviewed - Caregivers shall obtain 16 clock hours of training annually

Standard 830.B discussed - Shelter-in-place procedures shall be practiced a minimum of twice per year

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 12/10/2021 and concluded on 12/13/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 11 children present, ranging in ages from 7 months - 2 years, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 children's records and 2 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-90-A
Description: Based on record review and interview, the provider failed to ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Evidence:
1. Child 1, enrollment date 12/13/2021, lacked documentation of required immunizations.
2. The provider confirmed that immunization information was not on file for child 1.

Plan of Correction: I received them the next day.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider failed to ensure that areas of the family day home shall be maintained in a safe condition.

Evidence:
1. Child 1, age 2 years, was observed napping on a rest mat that had been placed in the doorway between the two child care rooms.
a. This door way is in the path toward the exit door out of the home.
2. A fire safety / evacuation hazard was created when child 2 was placed in this doorway area for nap.

Plan of Correction: I removed her as we was talking.

Standard #: 8VAC20-800-250-A
Description: Based on observation, the provider failed to ensure that hanging items including, but not limited to, window blind or curtain cords, appliance cords, and ropes shall be out of reach of children under five years of age.

Evidence:
1. The cord from the wall mounted television was observed dangling down the wall in the child care room during the virtual inspection conducted on 12/13/2021.
a. This cord was accessible to the children in care.

Plan of Correction: I will get a cover put over the cord.

Standard #: 8VAC20-800-590-B
Description: Based on observation and interview, the provider failed to ensure that an infant who falls asleep in a play space other than his own crib shall be moved promptly to his designated sleeping space if the safety of the infant is in question.

Evidence:
1. Child 1, age 8 months, was observed sleeping on a couch during the virtual inspection at approximately 1:30 pm.
a. Child 1 was sleeping on her tummy on top of a receiving blanket.
b. The provider stated that child 1 had the ability to roll over.
2. The provider stated child 1 had fallen asleep while being held by caregiver 2 and had then been placed on the couch to continue sleeping.
3. A couch is not a safe sleeping space for infants due to the risk that that the child could fall or roll off of the couch. Additionally, cushions are considered soft bedding which pose a risk of suffocation.

Plan of Correction: This I do not agree to. The infant wasn't sleep when taken out of crib. She fell asleep as we was on the phone. Didn't want to move her because she was going to cry. Infants always sleep in crib.
