Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interview and a virtual tour of the program.

A subsidy health and safety inspection (SHSI) was initiated on 12/21/2021 and concluded on 12/21/2021. There were 0 children present with 1 caregiver. A licensing inspection was conducted on the same date as the subsidy supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-400-A
Description: Based on record review, the provider did not ensure that the emergency preparedness plan addressed all required procedures.
Evidence:
The provider's emergency preparedness plan did not address caregiver responsibility and home readiness with respect to lockdown procedures.

Plan of Correction: I will update my emergency procedure plan.

Standard #: 8VAC20-790-190-B-12
Description: Based on record review, the provider did not ensure one of two children's records contained documentation of a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. The record of child #2 (start date: 7/5/21) did not have documentation of a written allergy care plan.
2. The record of child #2 had parent and physician documentation of a diagnosed food allergy.

Plan of Correction: I will call child #2's parent to get an allergy care plan completed from the doctor.
