Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an onsite tour of the program.
A renewal inspection was initiated on December 20, 2021 and concluded on December 20, 2021. An onsite inspection was conducted. There were seven children present, ranging in ages from 12 months to eight years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and two caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not ensure that all areas and furnishings of the family day home. inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence:
Child #7 (12 months of age) was observed in a portable infant swing. The infant swing had a broken right center frame with duct tape wrapped around the frame and the arm of the right side of swing. The right center frame was not completely attached to the arm of the right side of swing. The infant swing had blankets under the swing seat to prop the seat up.

Description: Based on a review of children's records, the center did not ensure that each child's record contains all required information.

Description: Based on observation the provider did not ensure that children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs.

Evidence:
1. In room #2, one set of stairs without a protective barrier to prevent access to the stairs was observed.
2. There were two infants and three toddlers at the family day home on the day of the inspection.

Description: Based on observation and interview, the provider did not ensure that infant walkers shall not be used.
Evidence: An infant walker was observed in room #1. The provider stated that the parent of an infant brought the infant walker to the family day home.

Standard #: 8VAC20-800-600-B
Description: Based on observation, the provider did not ensure that infants and toddlers spend no more than 30 minutes of consecutive time during waking hours, in a confining piece of equipment. The intervening time period between confinements shall be at least one hour.
Evidence: The inspector observed one child in an infant car seat for longer than 30 minutes while awake. The infant car seat is a seat designed specifically to protect children from injury or death during vehicle collisions.
