Technical Assistance:
1. Inspector provided training information.

Comments:
A subsidy health and safety inspection (SHSI) was conducted today to review supplemental health and safety requirements not covered in the licensing regulations. A licensing inspection was conducted on the same date, and the two inspections combined cover all SHSI requirements. Areas of non-compliance found of the supplemental subsidy health and safety requirements. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Standard #: 22VAC40-665-230-H
Description: Based on records requested, it was determined that caregivers have not completed a required annual (every 12 months) subsidy vendor training.

Evidence:

1. Provider - Date of last Virginia Health + Safety Training (VHST) certificate was 3/18/19.

2. Assistant #1 - Date of last VHST certificate was 4/6/20.

3. Assistant #2 - Date of last VHST certificate was 3/20/20.

Plan of Correction: Caregivers will complete the class in the next 30-days.
