Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, document review, and an interview with the provider. A monitoring inspection was initiated on 1/25/2022 and concluded on 1/26/2022 with an exit interview. The provider was contacted by telephone and a virtual inspection was conducted. There were six children present, ranging from nine months to five years of age, with the provider and an assistant supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical environment and equipment, care of children, preventing the spread of disease, medication administration, emergencies and nutrition. A total of two child records and two caregiver records were reviewed. The information gathered during the inspection determined non-compliance with one applicable standard or law and the violation was documented on the violation notice issued to the program. Description:	Based on review of two caregiver records, and an interview with the provider, it was determined that not all caregivers had a repeat Central Registry Search report every five years as required. Evidence: The record for caregiver #1 had a Central Registry Search report dated: 9/16/2016.
Plan of Correction:	Provider stated they will obtain an updated Central Registry Search report.
