This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
A monitoring inspection was initiated on 02/01/2022 and concluded on 02/03/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present, ranging in ages from 7 months to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff/household records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The record for Child #2 (start date 11/29/2021) did not contain documentation of a physical examination.Plan of Correction: Will request the parents to provide the physical examination.
