Comments:
This inspection was conducted by licensing staff using an alternate hybrid protocol.

A renewal inspection was initiated on 02/04/2022 and concluded on 02/08/2022. The provider submitted requested documentation to the inspector on 02/04/2022 for review and the inspector conducted an unannounced inspection on-site on 02/08/2022 from approximately 8:30am to 10:00am. On 02/08/2022, there were four children present with two caregivers supervising for a value of ten points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, two caregiver records and two household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-140-B-3
Description: Based on review of caregiver records and interview, the provider did not maintain current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

Evidence:

The CPR certification for caregiver #1 expired on 11/02/2021. Caregiver #1 stated she did not have current CPR as she had not renewed her CPR certification as of yet.

Plan of Correction: Per the provider: I have registered for a CPR class to be completed on 02/21/2022.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not ensure each caregiver and adult household member obtained a current Report of Tuberculosis Screening every two years from the date of the first screening.

Evidence:

Documentation of the most recent tuberculosis screening for household member #1 was dated 05/16/2019 exceeding the requirement to renew every two years.

Plan of Correction: Per the provider: The household member obtained an updated tuberculosis screening however did not have documentation of the screening. The provider will have the household member obtain the documentation and place it in the file.

Standard #: 8VAC20-800-210-A
Description: Based on review of training certificates, logs and interview, the provider did not ensure in addition to satisfactory completion of first aid training and CPR training, caregivers obtained a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:

The training log for caregiver #1 had a total of 15 hour of annual training for the period 05/31/2020 through 05/30/2021 when 16 hours was required.

Plan of Correction: Per the provider: I miss counted the number of hours obtained. I will ensure that 16 hours is completed every year.
