Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was conducted on January 28, 2022. The provider was contacted by telephone and a virtual inspection was conducted. There were three children present, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one caregiver record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	22.1-289.035-B-2
Description:	Based on review, provider did not ensure required background check is completed prior to caregiver beginning employment. Evidence: The record for caregiver #1, date of employment October 26, 2021, had documentation of a completed national fingerprint check dated November 4, 2021.
Plan of Correction:	In the future, a national fingerprint check will be completed prior to a caregiver beginning employment.
Standard #:	22.1-289.035-B-4
Description:	Based on review, provider did not ensure caregiver has required out of state background checks within the timelines required whenever a individual has indicated that they have lived outside the state of Virginia within the past five years. Evidence: The following was found during review of caregiver #1 record, start date of employment October 26, 2021: 1. Caregiver indicated she has lived outside the state of Virginia within the past five years. There was not a sex offender registry search for that state completed prior to beginning employment. 2. Caregiver indicated she has lived outside the state of Virginia within the past five years. There was not a child abuse and neglect search for that state completed within thirty days of beginning employment.
Plan of Correction:	Provider will first review to verify correct forms were submitted. If correct forms were submitted, she will follow up with that state to determine the status of the submissions. If correct forms were not submitted, new forms will be completed and submitted.

Description:	Based on review, provider did not ensure a caregiver record has documentation of required information. Evidence: The record for caregiver #2, start date October 26, 2021, did not have documentation of two reference checks.
Plan of Correction:	Provider will request two reference letters from the individual today.
