Technical Assistance:

1. Discussed ongoing health and safety practices as it relates to the health pandemic. 2. Inspector provided guidance for completing enrollment forms prior to giving them to parents. 3. The most recently issued Violation Notice should be posted in the daycare area of the home, typically near the State License. 4. Personal care items should be kept out of reach of children, and the storage of such items should be assessed as the children grow and can reach more areas.

Comments:

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time. An unannounced renewal inspection was conducted at 10:18am. The provider was contacted by phone to initiate the inspection. There were 2 children present = 4 points with the provider. Ratios and supervision were in compliance at the time of inspection. Children were observed playing with toys during the inspection. The home was sufficiently supplied with toys and equipment for children, and appeared clean and organized today. The provider completed annual training requirements. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Description:	Based on records reviewed, it was determined that 2 of 2 children's record did not include required information in writing prior to the child's first day of attendance: Evidence: 1. The records for Child #10 + #11 did not include information for parents listed in this regulation section: Operating hours, holidays, fees and payments, check-in and out procedures, medication policy, liability insurance, general daily schedule, discipline policy, provisions of the emergency preparedness plan, and termination policy. 2. While these forms were signed by parents, the forms and information were left blank by the provider.
Plan of Correction:	The forms and information will be completed by the provider, and re-signed and dated by the parents. In the future, the provider will give parents completed forms to sign.
