Technical Assistance:
Consultation provided on the need to do two shelter in place drills yearly.
Shelter in place drills are in addition to the monthly escape drills.
You must activate the smoke detector for your monthly drills

Comments:
An virtual renewal inspection was conducted today with the provider, who was present with 5 children, age ranging from 5 months to 3 years for a total of 16 points. One child had emergency medication onsite. The provider's Medication Administration Training for emergency medication was obtained on 3/15/21. The provider and the assistant's First Aid/CPR certification is through ASHI and is due to expire on 1/9/23. Three children's files were reviewed.
The violations issued are outlined on the violation notice.

Please provider me a copy of the central registry background check for Person A.

Violations:
Standard #: 22VAC40-665-410-A-2
Description: Based on interview, it was determined that the provider has not practiced shelter in place drills twice a year as required. There was no documentation of shelter in place drills practiced for the 2021 year.

Plan of Correction: I will do shelter in place drills twice a year and keep documentation.

Standard #: 8VAC20-770-40-D-5-a
Description: Based on interview, it was determined that a 16 year old household member (A) did not have a central registry results on file. This background check is required for household members when they turn 14 years of age.

Plan of Correction: I will get this background checks and send you the results.

Standard #: 8VAC20-800-180-A
Description: Based on interview and a review of the tuberculosis test/screenings, it was determined that two household member's tuberculosis tests/x-ray were more than 2 years old.
Evidence:
Person B's tuberculosis test is dated 11/13/19 and Person C's tuberculosis x-ray is dated 12/19/19.

Plan of Correction: We will obtain new tuberculosis test/x-rays.
