Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on February 18, 2022. The provider was contacted by telephone and a virtual inspection was conducted. In the care of the provider and two assistants were 10 children, ranging in age from 7 months through 4 years (27 Points). The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, training, care of children, physical environment, nutrition and emergencies. Two child records, the provider's record and the record of two assistants were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on the review of two children's records and verification from the provider, one child's record did not contain the contact information for a second emergency contact.

Evidence:
1. The record for Child #1 did not contain the name, address or phone number of a second person for the provider to call in case of an emergency and the parent of the child could not be reached. Date of enrollment was 05/24/2020.
2. The provider verified that the information was not in the child's file.

Plan of Correction: The provider reached out to the parent during the inspection and the name, address and phone number of a second emergency contact was obtained and put in the child's file.

Standard #: 8VAC20-800-180-A
Description: Based on the staff record review and discussion with the provider, two caregivers had not obtained a two year updated negative tuberculosis (TB) screening.

Evidence:
1. The most recent negative TB screening for Staff #1 was 02/10/2020.
2. The most recent negative TB screening for Staff #2 was 01/27/2020.
2, The provider confirmed that neither staff member had obtained an updated TB screening.

Plan of Correction: The provider will have both staff members obtain the updated TB screenings as soon as possible.
