Adult 1 did not have the results of a central registry check on file. The background check was submitted in the required time frame, but the results have not been received and documented follow up with the agency was not available for review on the date of inspection.Plan of Correction: I did send it again for the second time and it was expedited. I will inform you ASAP.
Standard #: 8VAC20-800-60-B Description: Based on record review, one of two children's records did not contain all required information.
Evidence:
Child 1's record did not contain addresses for two persons to contact in the event that parents cannot be reached.Plan of Correction: I already talked to the parent and got it done.
Standard #: 8VAC20-800-170-A Description: Based on record review, the provider did not obtain, at the time of hire, a negative Tuberculosis screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Evidence:
