Discussion of a CPS check needed prior to when a household member turns 14 years of age. A CPS check (a search of the central registry of Virginia) emailed to the FDH Provider.

Comments:
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and review of the program.

This inspection was initiated and concluded on 03/01/2022. The Provider was contacted by telephone and an inspection was conducted. Due to technical difficulties, the scheduled virtual inspection could not be administered.
There were 2 children present (1 child in care, and 1 resident child), ranging from toddler to school-age (4 points), with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 3 children records, 1 staff record, and 3 household records were reviewed.

Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the program.

Standard #: 8VAC20-800-90-C
Description: Based on review, 1 of 3 children files indicated that the Family Day Home did not obtain documentation of additional immunizations for a child who was not exempt from the immunization requirements (according to subsection B of this section) once every six months for all children under the age of two years.

Evidence -
1. On the date of inspection (03/01/2022), the most current immunization page on file for Child #1 was dated - 05/29/2021.
2. Based on the documentation on file, Child #1 was under the age of two years on the date of inspection.

Plan of Correction: The parent will be contacted to obtain an updated immunization page.
