Comments:
An unannounced renewal inspection was conducted today, March 1, 2022 from 9:00am until11:00am. There were 10 children present (28 pts) with 3 caregivers supervising. The children were observed eating breakfast, playing with floor toys and working on numbers on the computer. The physical plant, emergency plans, emergency supplies, bottles, cribs, evacuation drill logs, menu's , child and caregiver records were reviewed. Standard #: 8VAC20-770-40-D-1-b
Description: Based on a review of caregiver records, the Family Day Home does not have current Central Registry Background checks for caregivers.

Evidence:
Caregiver #1, #2 and #3's records do not contain documentation of current Central Registry Background checks. Caregiver #1's last central registry background check is dated02/06/2017, Caregiver #2's last central registry background check is dated 5/26/2016 and Caregiver #3's last central registry background check is dated 6/07/16.

Plan of Correction: I will submit requests for Central Registry Background checks for caregivers.

Standard #: 8VAC20-800-760-A-1
Description: Based on a review of the first aid kit, the Family Day Home's first aid kit did not contain all required items.

Evidence:
The first aid kit is missing 2 triangular bandages and a first aid manual.
The first aid kit did not contain a thermometer, 2 triangular bandages and the first aid manuel.

Plan of Correction: I will purchase these items and place them in the first aid kit.

Description: Based on a review of Evacuation Logs, the Family Day Home failed to practice Shelter-In-Place drills a minimum of twice per year.

Evidence:
There was no documentation of Shelter-In-Place drills completed within the last 12 months.

Plan of Correction: I will remember to complete and document the Shelter-In-Place drills at least 2 times per year

Description: Based on observation, the Family Day Home failed to have the menu posted in an area accessible to parents.

Evidence:
The Family Day Home did not have a menu posted.

Plan of Correction: The menu was posted during this inspection.

Standard #: 8VAC20-800-960-E
Description: Based on observation the Family Day Home did not have bottles refrigerated and labeled with the child's full name and the date, if more than one infant is in care.

Evidence:
The container with bottles was on the counter and three of the bottles were not labeled with the child's full name and dated.

Plan of Correction: We will remind parents to label and date bottles and we will put the name and date on the bottles if the parents forget.

Standard #: 8VAC20-800-180-A
Description: Based on a review of caregiver records, the Family Day Home failed to obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Evidence:
Caregiver #1, #2 and #3 do not have documentation of a current Tuberculosis Test or Screening in their records. Caregiver #1's last test was 2/15/2019. Caregiver #2's last test was 2/15/2019. Caregiver #3's last test was 2/08/2019.

Plan of Correction: I will have these caregivers obtain and submit the required tuberculosis test.

Standard #: 8VAC20-800-210-A
Description: Based on a review of training records, the Family Day Home failed to obtain 16 hours of training annually.

Evidence:
Caregiver records did not home documentation of training hours for 2021-2022

Plan of Correction: I will remind caregiver that they must obtain training hours annually.

Standard #: 8VAC20-800-280
Description: Based on observation the Family Day Home failed to ensure that poisonous materials are stored in an area that is not accessible to children.

Evidence:
A spray sanitizer that was 70% alcohol and stated keep out of reach of children was in a place that was accessible to children.

Plan of Correction: We will keep poisonous material out of reach of children.
