Technical Assistance:
The Virginia Department of Education (VDOE) assumed oversight of child care licensing and
monitoring on July 1, 2021. All associated regulations and codes have transferred over from the
Virginia Department of Social Services. Guidance, resources and training links can be found on the new website: childcareva.com.
The violations on the Violation Notice reflect the new VDOE codes and standards.

Comments:
A Monitoring inspection was initiated today. This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. The provider was contacted by telephone and a virtual inspection was conducted. There were a total of 11 children present with 3 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and all staff records and 2 household were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-830-A
Description: Based on review of the Provider's emergency evacuation record, The emergency evacuation procedures was not practiced monthly with all caregivers and children in care during all shifts that children are in care.
Evidence:
The most recent evacuation drill was conducted in July 2021.

Plan of Correction: We will conduct both a fire drill and a shelter in place drill this month. Fire drills will be conducted monthly.

Standard #: 8VAC20-800-830-B
Description: Based on review of the Provider's emergency response drills, Shelter-in-place procedures was not practiced a minimum of twice per year.
Evidence:
No Shelter in place drills were completed in 2021.

Plan of Correction: I will conduct a shelter in place drill this month and a second one before December 2022.

Standard #: 8VAC20-800-100-A
Description: Based on child record review, documentation of a physical exam was not obtained prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
Child #1 began care on 01/03/2022. A physical exam was not on file for this child.

Plan of Correction: I will request a copy of the most recent physical exam and maintain it in the record.

Standard #: 8VAC20-800-90-A
Description: Based on child record review, one child record was determined incomplete.
Evidence:
Child #1 began care on 01/03/2022. No record of immunizations were found.

Plan of Correction: I will obtain these documents from the parents.
