Technical Assistance:

The Virginia Department of Education (VDOE) assumed oversight of child care licensing and monitoring on July 1, 2021. All associated regulations and codes have transferred over from the Virginia Department of Social Services. The violations listed below reflect the new VDOE codes and standards. Discussed with provider: Residents (when background searches are required), safety gate usage at stairs.

Comments:

An in-person renewal inspection was conducted today. There were 4 children present (11 points) with one caregiver present. 4 child and 2 household member records were reviewed. See Violation Notice for missing information. The Emergency Preparedness Plan needs to be updated annually, and reflect the relocation address provided to parents in the child record. Emergency evacuation drills, first aid supplies, emergency supplies and menu are all in compliance. The inspection report will be typed off site and emailed to the provider for signatures. Description:	Based on a review of the Emergency Preparedness Plan (EPP), the plan has not been reviewed annually. Evidence: The EPP available for review, has the last update/review documented as 2019. (model form sent to provider).
Plan of Correction:	I will update the plan, including the new emergency relocation address.
Standard #:	8VAC20-800-110-A
Description:	Based on a review of records, one child record does not include all required information. Evidence: 2 of 4 records reviewed did not include a parent signature on the "School Entrance Health form" used. (Children #1, #3)
Plan of Correction:	I will ask the parents to complete the form and sign it, or provide a new one.

Description:	Based on a review of records, child records are not kept up to date. Evidence: 3 of 4 records reviewed did not have an annual review for updates. *Child 2 - record is dated 6/29/20 - expired *Child 3 - record is dated 8/16/20 - expired *Child 4 - record is dated 5/4/20 - expired
Plan of Correction:	I will ask the parents to review and update the records.

Description:	Based on a review of records, there is no written acknowledgment of all required policies and procedures, prior to enrollment, for one child. Evidence: 1 of 4 records reviewed did not contain documentation of the Declaration of Liability Insurance signed by the parent. (Child 1)
