Technical Assistance:
The Virginia Department of Education (VDOE) assumed oversight of child care licensing and monitoring on July 1, 2021. All associated regulations and codes have transferred over from the Virginia Department of Social Services.

Comments:
An unannounced complaint investigation was conducted jointly with the local DSS agency. The provider was present with 1 assistant staff member during the inspection for 16 points which is within ratios for supervision. Interviews, physical observation of the daycare space and record review was conducted. Based on the Information gathered during the inspection it was determined non-compliance with applicable standards or law. Violations were documented on the violation notice issued to the program.

Description: Based on record review and interview statements, an injury report was not completed for an injury at the Family Day Home.
Evidence:
Child #1 sustained an injury while at the Family Day Home but the provider failed to complete a written report that contains required information.
Evidence:
The parents did not receive a written report that included the following information: date and time of the injury, name of the child, circumstances of the injury, caregiver present and first aid taken, date and time parent was notified as well as how the parent was notified, future action tp prevent reoccurrence and 2 signatures.

Description: Based on statement from the provider and lack of report, the provider failed to notify the department within 24 hours of a serious injury while under the supervision of the provider.
Evidence:
Child #1 was diagnosed with a broken bone while under the care of the provider. The provider did not notify the Licensing Office or Department of the serious injury at any time.
