Technical Assistance:

Discussion of the Sworn Disclosure Statement (SDS); copies were returned to the Provider. These copies were originally sent in to the licensing office. A blank copy of the SDS was email to the Family Day Home (FDH).

Comments:

This monitoring inspection was initiated and concluded on 03/08/2022. The Provider was contacted by telephone in order to obtain preliminary documentation (virtual component) and an on-site inspection was conducted (In person component) between the hours of 1pm to 3 pm. There were 6 children present (14 points), ranging from toddler to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (no meds); special care and emergencies; and nutrition. A total of 6 children records, 1 staff record, and 3 household records were reviewed. Information gathered during the inspection determined that there was one (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the program. Description:	Based on review, 1 of 6 children files indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (03/08/2022), the listed medical insurance for Child #5 was not available for review. 2. This review was confirmed by the Provider on the date of inspection.
Plan of Correction:	This information will be obtained by the parents of Child #5. If Child #5 has no medical insurance, it will be noted within their file.
