Comments:
An unannounced renewal inspection was conducted today between the hours of 9:30 am through 10:45 am. There were a total of two children (4 points) observed with one caregiver. A total of two children's files and five staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring, puzzles, bathroom breaks, diapering, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice.

Violations:
Standard #: 8VAC20-770-40-D-4-a
Description: Based on record review, the provider did not obtain a Central Registry Results for household members.
Evidence:
1. Household Member #2's record did not contain documentation of a current Central Registry finding. The Central Registry results in the record was dated 01/03/2017 and expired on 01/03/2022.
2. Household Member #4's record did not contain documentation of a completed Central Registry search.

Plan of Correction: The provider will re-submit the Central Registry and send the results to the Licensing Inspector.

Standard #: 8VAC20-800-800-A
Description: Based on record review and interview, the family day home did not have a written emergency preparedness and response plan.
Evidence:
1. The family day home did not have documentation of a written emergency preparedness plan. Caregiver #1 stated that the plan was completed, but could not locate the plan.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #2's first date of enrollment was 01/13/2020. Child #2's record did not contain results from a physical examination.

Plan of Correction: I will get a physical examination document from the doctor's office and document the record.

Standard #: 8VAC20-800-50-A
Description: Based on observation, the family day home did not keep a record of the children in attendance.
Evidence:
1. At the time of the inspection, there were two children in attendance. Documentation of attendance for the children in care was not taken for the 03/09/2022.

Plan of Correction: Attendance will be taken when the children arrive to the daycare.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Household Member #3's record did not contain documentation of a subsequent Tuberculosis (TB)Screening. The TB screening in the record was dated 03/24/2017 and expired on 03/24/2019.

Plan of Correction: The Household Member has an appointment for a TB screening tomorrow. Once the results are sent, I will document the file and send results to the Licensing Inspector.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was not cleaned and sanitized after each use.
Evidence:
1. Caregiver #1 changed the diaper of a child in care. After the diaper change, Caregiver #1 washed their hands and the child's hands, but did not clean and sanitize the diapering surface after use.

Plan of Correction: I will clean and sanitize the diapering surface after each use.
