Technical Assistance:

Discussion that the facility has at least (2) two carbon monoxide detectors installed on-site. A blank CPS check form was sent to the Provider in order to complete the two requests that have expired and for the one set to expire in September.

Comments:

A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a discussion of the program. This inspection was initiated and concluded on 03/16/2022. The Provider was contacted by telephone and an inspection was conducted. There were 4 children present (12 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. All 4 children records, 1 staff record, and 2 household records were reviewed. Information gathered during the inspection determined that there was (1) one non-compliance with an applicable standard. Description:	Based on review, 2 of 3 staff and household files indicated that it has been more than (5) five years since the dates of the most recent central registry finding (CPS check) on file at the Family Day Home. Evidence - 1. On the date of inspection (03/16/2022), the CPS check on file for Staff #1 and Household Member #1 were both expired. 2. The date of the CPS check for Staff #1 was dated - 02/03/2017. The date of the CPS check for Household Member #1 was also dated - 02/03/2017.
Plan of Correction:	New requests will be sent in order to obtain updated findings. These requests will be sent within the next ten days for both individuals. Once these findings are received they will be shared with our Licensing Inspector.
