8VAC20-770 Background Checks for Child Day Programs and Family Day Systems

Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:00 am through 11:15 am. There were a total of seven children (16 points) observed with two caregivers. A total of three children's files and five staff/household member's files were reviewed. The children were engaged in the following: toy play, games, circle time, coloring, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-770-40-D-4-a
Description: Based on record review, two household members do not have a current Central Registry Finding.
Evidence:
1. Household Member #2's record contains an expired Central registry Finding dated 01/10/2017. The Central registry Finding expired on 01/10/2022.
2. Household Member #3's record contained an expired Central Registry Finding dated 12/06/2016. The Central Registry Finding expired on 12/06/2021.

Plan of Correction: The provider will conduct a Central Registry Finding for both Household Members and send the LI the results.

Standard #: 8VAC20-800-830-A
Description: Based on record review and interview, the family day home did not practice monthly emergency evacuation procedures.
Evidence:
1. Documentation for emergency evacuation drills were missing for the months of November, December, January, and February.
2. Staff # 1 stated that the emergency evacuation drills were conducted but not documented in the log.

Plan of Correction: The provider will ensure to document the log.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates was not be stored away in areas inaccessible to children.
Evidence:
1. Toilet cleaners, sanitizing wipes and other cleaning agents were found in a low cabinet in the bathroom without a lock.

Plan of Correction: The provider will put a lock on the cabinet.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child-resistant protective covers larger than 1-1/4 inches in diameter were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
1. Two electrical outlets were not covered with child resistant covers in the area where the children play.

Plan of Correction: The provider will put two electrical outlet covers on the electrical outlets.

Evidence:
1. The findings of the most recent inspection of the facility was not posted in the facility.
