Technical Assistance:
Discussion of the background check that has expired and how to obtain new findings through the VDOE website.

Discussion that the FDH Renewal Application has not yet been received. Please complete and submit as soon as possible.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 04/01/2022, between the hours of 10 am to 11:30 am. There were 10 children present (25 points), ranging from infant to preschool-age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 11 children records, 3 staff records, and 5 household records were reviewed.

Information gathered during the inspection determined that there were two (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the program.

Violations:
Standard #: 8VAC20-770-40-D-1-a
Description: Based on review, 2 of 3 staff files, and 2 of 5 household files indicated that the searches of the central registry (CPS checks) on file at the Family Day Home (FDH) were older than five (5) years.

Evidence -
1. On the date of inspection the current CPS checks on file at were as follows: Staff #1 (dated - 04/28/2016), Staff #3 (dated - 05/09/16), Household Member #1 (dated - 05/09/2016), and Household Member #2 (dated - 05/09/2016).
2. The Provider mistakenly requested the wrong background check (for all four individuals) through the Virginia State Police and not through a search of the central registry.

Plan of Correction: New CPS checks will be conducted through the online portal. Once these new findings are obtained, they will be shared with the Licensing Inspector.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation the most recent violation notice of the FDH was not posted.

Evidence -
1. On the date of inspection (04/01/2022), the Violation Notice issued for an inspection conducted on 10/19/2021 was not posted.
2. Only the Inspection Summary for that inspection was posted.

Plan of Correction: When a Violation Notice is issued, it will be posted on the main bulletin board.
