Technical Assistance:
The following was discussed with the provider:
Shelter-in-place drills must be clearly documented as shelter-in-place drills to differentiate them from evacuation drills.
When 2 or more siblings are enrolled, a complete set of ALL required documents is required in each child's file.
There are wires along the side wall of the house that have been taped to be inaccessible to the children. The tape is disintegrating and must be replaced.
First aid kits must contain more than one triangular bandage.
The provider stated that no children have been transported in the family vehicles, however if a child(ren) is transported, required permissions and car seats will be obtained/used.

Comments:
A monitoring inspection was conducted on 4/5/2022. There were 6 children present, ranging in ages from 13 months to 3 years, and totaling 12 points, in care with the provider. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 4 caregiver/household records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-770
Description: Based upon observation and provider interview, the provider has not ensured that there is a working battery-operated weather band radio.
Evidence:
1. The provider produced a battery operated radio, however there were no batteries in the radio.
2. The provider verified that she did not have the needed batteries (C cell) to fit the radio.

Plan of Correction: The provider responded with the following during the inspection:
C cell batteries will be purchased and maintained for potential use in the radio.

Standard #: 8VAC20-800-240-A
Description: Based upon observation and provider interview, the provider has not ensured that areas and furnishings of the family day home are maintained to be clean and safe.
Evidence:
1. The plastic coated pad on the diaper changing table is torn, making it impossible to effective sanitize the table between diaper changes.
2. The provider acknowledged that the diaper changing pad is torn.
3. There were two climbing structures with slides on the backyard play area. Th rubber mats under the climbing structures were no longer connected, revealing hard packed ground beneath them. The climbing structures were positioned with the slides facing each other in a way that children could collide with each other at the bottoms of the slides.

Plan of Correction: The provider responded with the following during the inspection:
A new diaper changing pad will be purchased.
The rubber mats will be re-connected and the slides will be positioned with the slide exit chutes facing away from each other.

Standard #: 8VAC20-800-250-E
Description: Based upon observation, the provider has not ensured that latex gloves are inaccessible to children under five years of age.
Evidence:
There was a container of latex gloves on the shelf under the table used for diaper changing. All six children in care were under five years of age.

Plan of Correction: The provider responded with the following during the inspection:
The container of latex gloves was immediately moved into the bathroom to a cabinet that is inaccessible to the children in care.

Standard #: 8VAC20-800-320-A
Description: The provider has not ensured that small electrical appliances are unplugged when not in use.
Evidence:
There was an air fryer and other small appliances that were plugged into the wall and were not being used by the caregiver.

Plan of Correction: The provider responded with the following during the inspection:
All kitchen appliances will be unplugged when not in use while the children are in care.

Standard #: 8VAC20-800-320-G
Description: Based upon observation, review of documentation and provider interview, the provider has not ensured that fireplaces are inspected annually by a knowledgeable inspector who verified that the fireplace is properly installed, maintained and cleaning as needed.
1. There is a fireplace in the room designated as the child care room. There was no documentation of a fireplace inspection within the past year.
2. The provider verified that the fireplace had not been inspected within the past year.

Plan of Correction: The provider responded with the following during the inspection:
The fireplace is never used, however an inspection of the fireplace will be obtained.
