Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documentation review, and interview.

A monitoring inspection was initiated on 3/10/22 and concluded on 3/29/22. The provider was contacted by telephone and a virtual inspection was conducted. There were three children present, all three-years-old, with the provider. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, special care and emergencies, and nutrition. One child record, the provider record, and two household records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-140-B-3
Description: Based on record review and interview, the provider did not maintain current certification in cardiopulmonary resuscitation (CPR). Evidence: The CPR certificate on record for the provider was dated 2/10/20 and expired on 2/10/22.

Plan of Correction: I have signed up for a class on 4/16/22.

Standard #: 8VAC20-800-210-A
Description: Based on review of documentation and interview with the provider, the provider did not ensure that all caregivers completed a minimum of 16 hours of training annually. Evidence: Annual training was requested as part of the inspection. The provider did not submit any training certificates for the past year.

Plan of Correction: I will complete 32 hours of training this year to make up for last year.
