Technical Assistance:
Discussion of the role of Household Member #1. Once HM #1 becomes the Provider's assistant, he would need to obtain 16 hours on annual training, and complete all other training requirements. Background checks for HM #1 are all current and up to date for the Family Day Home (FDH).

A copy of the FDH Renewal Application, Assistant / Substitute Provider Record form, and Orientation form was sent to the FDH.

The Emergency Preparedness & Response Plan (EP&RP) was posted and dated on the parent bulletin board. Please continue to sign, and date an annual review of this document.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 04/06/2022, between the hours of 10 am to 12:30 pm. There were 4 children present (10 points), ranging from infant to preschool-age, with 1 caregiver and 1 volunteer supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 6 children records, and 1 household record, and 1 staff record was reviewed.

Information gathered during the inspection determined that there were three (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the program.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on review, 1 of 6 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.

Evidence -
1. On the date of inspection (04/06/2022), documentation of a physical examination was not available for review for Child #1.
2. The start date for Child #1 was listed as 05/27/2019. Child #1 was in attendance on the date of inspection.

Plan of Correction: Documentation of a physical examination will be obtained for Child #1.

Standard #: 8VAC20-800-90-C
Description: Based on review, 1 of 6 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (04/06/2022), the most recent immunization page on file for Child #6 was dated - 07/21/2021.
2. Based on information contain on file, on the date of inspection, Child #6 was under two years of age.

Plan of Correction: An updated immunization page will be obtained for Child #6.

Standard #: 8VAC20-800-180-A
Description: Based on review, 1 of 1 staff file, and 1 of 1 household file indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most recent TB documentation on file.

Evidence -
1. On the date of inspection (04/06/2022), the TB documentation on file for Staff #1 was dated - 06/22/2016.
2. The TB documentation on file for Household member #1 was dated - 02/12/2018.

Plan of Correction: Updated TB documentation will be obtained for both Staff #1 and Household Member #1.
