Technical Assistance:
Inspector had a conversation with the provider regarding mobiles over cribs and lack of appropriate diaper changing pad covers.

Comments:
An unannounced monitoring inspection was conducted today between 9:30 a.m. and 10:30 a.m. There were 10 children present with the Provider and 1 assistant for a total of 22 points within the supervision guidelines. The children were observed playing with chalk, riding bikes, eating snacks and playing freely. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, and menu were observed during this inspection. Thank you for your cooperation. Standard #: 22.1-289.036-A
Description: Based on record review and interview, the applicant did not ensure that a central registry background check was completed for each caregiver and/or household member every five years.
Evidence: The central registry background check for Caregiver #1, Caregiver #2, Household Member #1 and Household Member #2 expired on 04/11/2022. The provided stated that she submitted the background checks for review and February, but the results have not been received. There was no documentation that a follow-up was made.

Description: Based on review of medication authorizations and interview, medication was not returned to a parent within 14 days of the authorization expiring.
Evidence: Child #1 has emergency medication kept on site. The parent's authorization for the medication to be administered expired on 10/01/2021. The provider stated that the medication was present.

Evidence: Several children were observed walking, playing and jumping with ice pops, croissants and squeeze pouches.
