An unannounced complaint inspection was conducted on 3/9/2022 to investigate an allegation received by the department related to licensed capacity. Based on evidence gathered through observations, interviews and documentation review, the complaint was determined to be valid. An exit interview was conducted on 4/6/2022.

Standard #: 8VAC20-800-40-A Complaint related: Yes Description: Based on observation, multiple interviews, and a review of documentation, it was determined that the provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity which is 12 children. Evidence: On 3/9/2022, between 3:35 pm and 4:30 pm there were 15 children in care. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-800-50-A Complaint related: No Description: Based on observation, multiple interviews, and a review of documentation, it was determined that not all children were included on the written attendance record. Evidence: on 3/9/2022, eight school age children were observed getting out of the day care van and going into the day care with a caregiver. Providers acknowledged that the eight children are enrolled and transported from the local elementary school. Only five of the eight school-age children were included on the written attendance record. Child #1, child #2, and child #3 were not listed on the attendance record.
