Technical Assistance:

Inspector will email child record model forms to applicant. The provider had a mesh sided pack N play which she intended to use as a crib. She will purchase a crib that meets the intent of the standards. Applicant understands she can not be licensed for infants until a crib is purchased.

Comments:

An initial inspection was initiated on 4/12/2022 and concluded on 4/12/2022. The 1 level apartment located on the second floor was inspected, the living room is used for the play area. Children will not have access to the apartment balcony. The apartment complex playground will be used for outside play. There were no children in care. The inspector reviewed compliance in the areas of personnel, physical environment, medication and emergencies .Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the program. Description:	Based on observation and applicant interview, the first aid kit did not contain triangular bandages. Evidence: The first aid kit did not contain triangular bandages.
Plan of Correction:	Applicant will purchase triangular bandages.
Standard #:	8VAC20-800-770
Description:	Based on observation and applicant interview, a working portable battery-operated weather band radio was not available. Evidence: The applicant did not have a portable battery-operated weather band radio.
Plan of Correction:	The applicant will purchase the correct portable battery-operated weather band radio.
Standard #:	8VAC20-800-240-A
Description:	Based on observation, areas and furnishings of the family day home, inside was not maintained in a clean, safe, and operable condition. Evidence: An elliptical machine was observed in the corner of the play area, a child could very easily pull the elliptical on themselves. A wall plate located in the play area was found pulled away from the wall, exposing wires. Cable wires were hanging from the wall in the play area which were accessible to children.
Plan of Correction:	Applicant will move Elliptical machine out of the play area. The wall plate will be secured to the wall and the cable wires will be secured out of the reach of children.

Description:	Based on observation, potentially poisonous substances, such as cleaning agents, disinfectants, deodorizers, were not inaccessible to children. Evidence: The door to the laundry room and bathroom cabinets were found unlocked with poisonous substances and cleaning agents inside and accessible to children.
Plan of Correction:	Applicant will purchase child safety locks for the laundry room door and bathroom cabinets.

Description:	Based on observation, sharp kitchen utensils were accessible to children. Evidence: Knives were observed in the unlocked kitchen cabinets.
Plan of Correction:	Applicant will purchase a child gate that will be placed at the kitchen entrance.

Description:	Based on observation and provider interview, a landline telephone, was not available and operable. Evidence: The applicant does not have a landline phone.
Plan of Correction:	The applicant will purchase a landline phone and ensure she has service.
