Comments:

An unannounced renewal inspection was initiated and concluded on 4/14/2022. The inspector was on site from 9:28- 10:40 am. There was 1 child in care with 1 caregiver. The inspector reviewed compliance in the areas of administration, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. 5 child records and 2 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description:	Based on review of five children's records and interview, the provider did not obtain documentation that one child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the first date of attendance as required. Evidence: 1. The record of child #1 (DOE:4/11/2022) did not contain an immunization record. 2. The provider acknowledged that the record had not been obtained prior to attendance.

Standard #:	8VAC20-800-240-A
Description:	Based on observation and interview, the provider did not ensure that all areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Evidence: 1. Broken cement was observed at the top of the stairs that led from the outdoor playground to the entrance to the home that the children utilized daily. The cracked areas posed a tripping hazard. 2. Rust was observed on the outdoor basketball hoop pole. 3. The provider acknowledged that the areas posed safety risks and were on the list to repair.
