1. Discussed ongoing health and safety practices as related to the health pandemic.
2. Floor spaces where children walk should be free of household items, ie. at the top of the stairs leading to the bathroom.
- Comments:

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time.
An unannounced monitoring inspection was conducted at 10:05am. The provider was contacted by phone to initiate the inspection. There was 1 child present = 2
points with the provider. Ratios and supervision were in compliance at the time of inspection. The home was sufficiently supplied with toys and equipment for children, and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Standard #: 8VAC20-800-100-A Description: Based on records reviewed, it was determined that 2 of 2 children's records did not include a physical exam within the first 30-days of attendance.
Evidence:
1. Child #2 - There was no physical exam available for review. The child's 1st day of attendance was 7/18/19.
2. Child #3 - There was no physical exam available for review. The child's 1st day of attendance was 5/7/19.Plan of Correction: The provider said she has requested the physical exams from parents and expects to receive them by the end of the week.
Standard #: 8VAC20-800-90-C Description: Based on records reviewed, it was determined that 1 of 2 children's records did not include immunizations updated every 6 months until the age of 2 years old. Evidence: Child #3 - The immunizations were dated May 2020, when the child was 15 months old. Plan of Correction: The provider said she has requested updated immunizations from parent.
