Comments:
An unannounced monitoring inspection was conducted at 12:50pm. There were 7 children present = 11 points with the provider + assistant #3. Ratios and supervision were in compliance at the time of inspection. Children were napping during the inspection. The home was sufficiently supplied with toys and equipment for children, and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Violations:
Standard #: 8VAC20-800-60-B
Description: Based on records reviewed, it was determined that 1 of 3 children's records did not include required information. Evidence: Child #15 - There was no documentation of the child's medical insurance, to include name and policy number.

Plan of Correction: Parent will be asked to complete this information on the enrollment form.

Standard #: 8VAC20-800-90-C
Description: Based on records reviewed, it was determined that 2 of 3 children's records did not include immunizations every 6 months until the age of 2 years old and again between the ages of 4 years old and 6 years old.

Evidence:

1. Child #4 - The immunizations were dated February 2020, when the child was 7 months old.

2. Child #15 - The immunizations were dated July 2014, when the child was 21 months old.

Plan of Correction: The provider will review children's records to ensure immunizations are up-to-date on an ongoing basis.
