Technical assistance was offered regarding children's records and staff trainings.
- Comments:

A monitoring inspection was conducted on 4/15/22. The director was contacted by telephone and a virtual inspection was conducted. There were 12 children (34 points) present with 3 staff supervising. A walk through of the family day home was conducted. The children were observed napping in cribs and cots. There were no medications on site. Infant bottles, children's equipment and daycare documents were reviewed. 3 children's records and 3 caregiver's records were reviewed.

Evidence: Child # 1's record was missing the parent's employment information and the medical insurance policy number.Plan of Correction: The information will be collected from the parent and the record will be updated.
