Technical Assistance:
Inspector and provider discussed practicing emergency procedures, subsidy requirements, and maintaining up to date records.

Comments:
An in-person renewal inspection was conducted on April 19, 2022. There was 1 child present with 1 staff supervising. The child was observed watching television. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record, 1 staff record and 1 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-830-A
Description: Based on record review, the emergency procedures were not practiced monthly.

Evidence: There is no documentation of drill being conducted in January, February and March 2022.

Plan of Correction: I will practice drills and write them down.

Standard #: 8VAC20-800-830-B
Description: Based on interview and record review, the provider has not practiced the shelter-in-place procedures at a minimum of twice per year.

Evidence: The provider stated they have not practiced the shelter-in-place procedures. No shelter-in -place was documented on their emergency drill record form.

Plan of Correction: I will make sure to practice shelter in place procedures.

Standard #: 8VAC20-800-60-B
Description: Based on record review, the child's record did not contain all the required information.

Evidence: Child 1 is missing second contact information and file was not reviewed annually to ensure all information is up to date.

Plan of Correction: I spoke with parent previously and requested a second emergency contact. I will ask the parent again.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not ensure all tuberculosis screenings were repeated every two years from the date of the most screening.

Evidence: Caregiver 1 has a tuberculosis screening which was completed on April 23, 2018. Household member 1 has a tuberculosis screening was completed on April 23, 2018.

Plan of Correction: We will do repeat checks as soon as possible.
