Technical Assistance:
Crib sheets must be tight fitting when an infant uses a crib. The crib was not in use before a tight fitting sheet was placed on the mattress.

Comments:
An in person monitoring inspection was conducted today. There were 6 children present (15 points) with 2 caregivers present. 5 child and 7 caregiver/household members records were reviewed. See Violation Notice for missing items. Outside time discussed with the provider: front yard use while a shed is being built, or a General Fieldtrip permission form would be required to take walks in the neighborhood. The inspection will be typed off site and emailed to the provider for signatures. LI was present today from 10:10 AM - 11:30 AM. Standard #: 22.1-289.035-A
Description: Based on a review of records, caregiver and household records do not contain documentation of repeated background searches within the allowable 5 year time frame.
Evidence:
6 of 7 records reviewed did not contain documentation of repeated background searches within 5 years.
*Central Registry Searches (CPS) for caregivers and household members are all dated 1/11/2017 (expired). (Caregivers A,B Household members C,D,E,F)
*Sworn Disclosure Statements (SDS) for caregivers and household members are expired.
(Caregivers A, B; SDSs are dated 12/25/2016. Household member C; SDS is dated 12/26/2016, Household members D,E,F - SDSs are dated 12/25/2016)

Plan of Correction: CPS searches will be submitted immediately.
SDS forms will be completed immediately

Standard #: 8VAC20-800-750-A
Description: Based on provider statement, a topical ointment in the facility is not labeled with the child's name. The topical ointment does not have a written authorization available for review.
Evidence:
*A tube of Aquaphor lotion in use for one child did not have a written authorization to apply a non-prescription topical skin product.
*The container was not labeled with the child's name to show ownership. (Child 5)

Plan of Correction: *The container was labeled during the inspection.
*I will ask the parent to complete the authorization form.

Standard #: 8VAC20-800-940-A
Description: Based on observation, the freezer temperature is not maintained at or below 0 degrees.
Evidence:
The freezer temperature registered at 3 degrees during the inspection.

Plan of Correction: The temperature will be lowered on the freezer.

Standard #: 8VAC20-800-50-A
Description: Based on observation, the provider has not maintained a written record of attendance each day.
Evidence:
At LI arrival (10:10 AM) the attendance sheet at the front door reflects that no attendance has been kept since 4/7/2022.

Plan of Correction: The attendance sheet was updated during the inspection, and will be kept current

Standard #: 8VAC20-800-60-B
Description: Based on a review of records, child records do not contain documentation of all required components.
Evidence:
5 child records were reviewed. Missing information includes:
*1 of 5 records was missing complete Father's information (Child 1)
*1 of 5 records was missing health insurance information (Child 3)

Plan of Correction: Parents will be asked to complete the missing information today.

Standard #: 8VAC20-800-70-A
Description: Based on a review of records, child records do not contain documentation of written acknowledgement of all required policies and procedures prior to enrollment.
Evidence:
5 child records were reviewed.
*Missing information includes but is not limited to: policies for drop off/pick up, payment due date, custodial rights statement, mandated reporter requirements, schedule of the day, termination policies, first aid notifications, liability insurance declaration, policy for administration of medication, provisions of the emergency preparedness plan.
