Comments:
An unannounced monitoring inspection was conducted today form 7:00am - 8:30 am. There were 2 (7 pts.) children present with one caregiver. The physical plant, staff records, children's records, emergency supplies, menu's bottles and household member's records were reviewed. The children were observed hand washing and free play. There was an abundance of books, toys and games for the age and developmental stages served by the provider. Areas of non-compliance were cited on the violation notice. Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the provider failed to ensure that the first aid kit contained all required supplies.

Evidence:
Missing from the kit were 2 triangular bandages and adhesive tape.

Plan of Correction: I will purchase these items.

Standard #: 8VAC20-800-830-A
Description: Based on a review of evacuation log, the provider has failed to practice monthly an emergency evacuation drill with all caregivers and children in care.

Evidence:
The last documented emergency evacuation drill was conducted in November 2021.

Plan of Correction: I will restart evacuation drills this month and I will document them each month

Standard #: 8VAC20-800-830-B
Description: Based on a review of evacuation logs, the provider failed to ensure that shelter in place procedures shall be practiced a minimum of twice per year.

Plan of Correction: I will complete the shelter in place practice procedure two times per year and I will document it.

Standard #: 8VAC20-800-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of a physical prior to the child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #1's record does not have documentation of a physical examination.

Plan of Correction: I will request form from parents.

Standard #: 8VAC20-800-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation of adequate immunizations before a child may attend the family day home.

Evidence:
Child #1's record does not have documentation of immunizations by the first day of attendance.

Plan of Correction: Requested from parents, I will request it again.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider failed to ensure that potentially poisonous substances shall be stored away in areas inaccessible to children.

Evidence:
Clorox Wipes were left on the desk.

Plan of Correction: Item was put in inaccessible area during inspection.
