Technical Assistance:

Discussed the need to maintain point structure for staff. Staff should not exceed a total of 16 points each. Discussed the need to change the transitions of care such as diapering and responding to children sleeping on a second level to ensure adequate supervision is maintained. Staff annual training will be reviewed at the next licensing inspection. Be sure to review the expiration dates for the topical ointments that are onsite.

Comments:

An unannounced Renewal Inspection was conducted today. A total of 12 children were present (32points) with one child belonging to the Provider. with 2 staff and 1 volunteer. Observation of the backyard, programming space, library, diapering area and the upstairs crib room was made. The Provider has implemented a great deal of Covid precautions to include an outdoor sink so that children can wash their hands before entering the home. No medication needs identified. Some topical ointments onsite. No identified food allergies for the children currently in care. A total of 4 children's records, all staff and 1 volunteer record were reviewed. Standard #:	8VAC20-800-560-A
Description:	Based on observation of the family day home, children were not always within sight of a caregiver. Evidence: 1. Staff #2 placed Child #1 at the feeding table and went upstairs to get Child #2. Child #1 was out of both sight and sound supervision for approximately 1 to 2 minutes. 2. Staff #2 proceeded to diaper Child #2. During this time, Child #1 was out of sight supervision for approximately 5 minutes. 3. After diapering Child #2, Staff #1 placed Child #2 at the feeding table and went upstairs to get Child #3. 4. During this time, Child #1and #2 were out of sight and sound supervision for approximately 1 to 2 minutes. 5. Staff #2 proceeded to diaper Child #3, leaving Child #1 and #2 out of sight supervision for approximately 5 minutes.
Plan of Correction:	Though the children were placed in a huge/sturdy commercial grade daycare feeding table and all were securely buckled up without anything in front of them or within reach that could pose chocking hazard or hurt each other. This is in addition to a live camera & a baby monitor pointing directly on them to monitor them all the time. It was brought to our attention that we can only use monitors while children are sleeping. Thus making this practice short handed and we will: (1st) Happily reconstruct our daily schedule of outdoor play & transitions to make sure that all the children are within sights and sounds all the time. (2nd) Lined up some jumperoo or bouncers like chairs right in front of the changing table. So, all infants can be seated there while waiting for their turn and after their turn and can be within sight and sounds all the time.
Standard #:	8VAC20-800-570-A
Description:	Based on observation, the Provider failed to ensure that a caregiver did not exceed 16points. Evidence: 1. There were 9 children (20 points) including the Provider's own child outside playing in the backyard under the supervision of the Provider and Volunteer #1. 2. The Provider was observed coming inside the home for approximately 1 or 2 minutes leaving Volunteer #1 alone with the 9 children (20 points). 3. During this time, Child # 4 (17 months) was observed climbing and standing on a plastic child picnic table. Volunteer #1 was unaware the child was standing on the table.
Plan of Correction:	Under normal circumstances the children and staffs are always in ratio. The excitement of today?s inspection and the need for the provider to quickly go back inside to turn over documentation for reviewing has lead to another staff being alone with 1 child in excess of her ratio and accidentally unaware of this kid climbing on a small toddler table. Though we worked hard on making sure that the surroundings are safe and all play structures & tables are age appropriate and necessary for all the kids fine & gross development. Including learning risk assessment abilities. We?ll make sure to re-train all staffs on proper supervision, alertness & will work on more effective way of engaging with kids and supervising everyone?s safety.
