Comments:
An in-person monitoring inspection was conducted today. There are 7 children (20 points) with 2 caregivers present today. 2 caregiver and 7 child records were reviewed. Violations discussed with the provider: infant with a crib while lying down, resilient surfacing mats under swing and slide exit, emergency preparedness plan updated, child record components. Standard #: 8VAC20-800-800-B
Description: Based on a review of documentation, the Emergency Preparedness Plan has not been reviewed annually.
Evidence:
The Emergency Preparedness plan is dated 1/21/21 (expired).

Plan of Correction: I will review and update the plan.

Standard #: 8VAC20-800-900-B
Description: Based on observation, a child was allowed to drink while lying down.
Evidence:
At LI arrival, an infant was observed lying down on in a crib while drinking from a bottle.

Plan of Correction: I will not allow children to drink bottles while lying down.

Standard #: 8VAC20-800-60-A
Description: Based on a review of records, child records are not maintained as up-to-date.
Evidence:
2 of 7 records reviewed were not updated annually.
*Child 2 - record is dated 1/20/21 (expired)
*Child 7 - record is dated 1/5/21 (expired)

Plan of Correction: I will have all of the records reviewed and updated.

Description: Based on a review of records, child records do not contain all required components.
Evidence:
7 records reviewed. Missing documentation includes:
*1of 7 records was missing physician's information. (Child 6)
*1 of 7 records was missing 2 emergency contact's complete address (Child 5)
*1 of 7 records was missing documentation of Proof of Birth within 7 days of enrollment. (Child 4: Date of enrollment: 3/21/22)

Plan of Correction: I will ask the parents to update the forms, including the missing information.

Standard #: 8VAC20-800-90-A
Description: Based on a review of records, child records do not include documentation of immunizations prior to the first day of attendance.
Evidence:
7 child records were reviewed.
2 of 7 records was missing documentation of immunizations.
Child #2 first date of attendance: 1/5/21. No immunizations are on file.
Child #6 first date of attendance: 4/12/22. No immunizations are on file.

Plan of Correction: I will ask the parent to bring me the shot records.

Standard #: 8VAC20-800-500-C
Description: Based on observation, the outdoor play equipment is installed over a hard surface.
Evidence:
The outdoor play equipment structure is installed on a a wooden deck covered with AstroTurf. There is no resilient surfacing under the swings or at the slide exit.

Plan of Correction: I will purchase resilient mats for under the equipment.
