A renewal inspection was conducted on April 28,2022 with two caregivers. There was 1 child present, age 8 months, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 1 household member record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-770-40-D-4-a Description: Based on caregiver interview, the provider failed to have a sworn disclosure statement for one household member within 30 days of turning 18 years old. Evidence: Household member 2 did not have documentation of a sworn disclosure statement. Plan of Correction: Caregiver 2 stated that household member 2 will complete a sworn disclosure statement today.
Standard #: 8VAC20-800-530-F Description: Based on observation, the facility failed to prevent use of soft bedding around infants at any time. Evidence: there was a blanket covering the entire car seat that Infant 1 was strapped into. Plan of Correction: Caregiver removed the blanket that was over the car seat.
