Comments:

A renewal inspection was conducted on 04/26/2022 between the hours of approximately 9:15 am and 10:45 am. There were 8 children present, ranging in ages from 2 to 5 years old, with the provider. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 1 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-800-50-A
Description:	Based on documentation review, the fdh did not keep a complete written record of children in attendance each day. Evidence: 1) On the date of inspection, there were 8 children in attendance. The attendance record for the day had only 4 names of children documented as present for the day.
Plan of Correction:	I will tell them to sign everyday.
Standard #:	8VAC20-800-60-A
Description:	Based on record review and interview with the provider, the provider did not maintain an up-to-date record at the family day home for each enrolled child. Evidence: 1) On the date of inspection, a record for Child #5 was not available for review. 2) The provider could not locate the record for Child #5.
Plan of Correction:	I already have. I confused with her sister that used to be here.
Standard #:	8VAC20-800-210-A
Description:	Based on documentation review and interview with the provider, the provider did not ensure to obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Evidence: 1) On the date of inspection, only 1.5 hours of training hours were documented for the year 2021. 2) The provider stated that it was the only training that she could complete in 2021.

Standard #:	8VAC20-800-320-A
Description:	Based on observation, the provider did not ensure that small electrical appliances such as, but not limited to, curling irons, toasters, blenders, can openers, and irons are unplugged unless being used by the caregiver or with children under close supervision. Evidence: 1) On the date of inspection, two small appliances were observed that were plugged in but not in use: a coffee maker machine in the kitchen area, and a flat iron in an upstairs bathroom area.
Plan of Correction:	Now are unplugged.

Description:	Based on documentation review, the provider did not ensure that wood burning fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Evidence: 1) The most recent fireplace inspection was dated 12/14/2019.
Plan of Correction:	I already called to inspection.
Standard #:	8VAC20-820-120-E-2
Description:	Based on review of required posted documents, the provider did not ensure that all required documents related to the terms of the license were posted on the premises of the program. Evidence: 1) On the date of inspection, the findings of the most recent inspection of the facility (11/2021) were not posted. The inspection report posted was dated 04/14/2021.
