Comments:
An unannounced inspection was conducted today. There were 9 children (22 points) present with 3 caregivers. The physical plant, children's records, staff records, first aid kit, cribs, bottles, topical creams, menu's, evacuation records and emergency preparedness plans were reviewed. The children were observed handwashing, singing songs and working on art projects. Areas of non-compliance are cited on the violation notice. Standard #: 8VAC20-800-780-B
Description: Based on a review of children's records, the provider failed to annually review with the parent the emergency contact information to ensure the information is correct and obtain the parent's acknowledgment of the review.

Evidence:
Child #1- (First date of attendance 7/2/2019 last review 09/03/2020), #2 - (First date of attendance - 02/03/2020),#3 - (First date of attendance 08/27/2020) and #4's - (First date of attendance - 04/02/2018, last acknowledged review 10/19/2019) records did not contain documentation of an annual review and acknowledgement of emergency contact information.

Plan of Correction: I will remember to complete annual review of emergency contact information.

Standard #: 8VAC20-800-100-A
Description: Based on a review of children's records, the provider failed to ensure that each child has documentation of a physical examination by the 30th day of attendance.

Evidence:
Child #2 and child #4's records do not have documentation of a physical by the 30th day of attendance. Child #2's first day of attendance was 02/03/2020 and Child #4's first day of attenance was 04/02/2018.

Plan of Correction: I will make sure that all new children will have proof of a physical by their 30th day of attendance.

Standard #: 8VAC20-800-60-A
Description: Based on a review of children's records, the provider failed to obtain all required documentation.

Evidence:
Child #3's record does not contain documentation of the names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will get this information from parent.

Standard #: 8VAC20-800-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation of immunization by the first day of attendance.

Evidence:
Child #4's record does not contain documentation of immunizations by the first day of attendance. The first day of attendance was 04/02/2018

Plan of Correction: I will make sure that each child has proof of immunizations on the first day of attendance.

Standard #: 8VAC20-800-480-E
Description: Based on observation, the provider failed to ensure that equipment used by children shall be clean and in good condition.

Evidence:
Cribs have chipping and peeling paint.

Plan of Correction: We will work on sanding and/or repaint.
