Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on May 2, 2022 from 11:40 a.m. through 12:45 p.m. Upon arrival, there were 3 children in the care of the provider and assistant. Children ranged in age from 10 months through 4 with a total of 10 points. The records for 3 children, the provider and 1 assistant were reviewed and updated. No medication is being administered at this time.
Areas observed were: handwashing/bathroom procedures, coloring activity, outside play, lunch preparation and lunch. Children appear to be well aware of the daily routine and schedule.

Standard #: 8VAC20-800-100-A Description: Based on record review, the provider did not obtain physicals for 3 children within 30 days of the first day of attendance.
Evidence:
1. Three children records were reviewed.
2. Child #1, #2 and #3 did not have documentation of obtaining a physical within 30 days of the start of attendance.
3. Child #1 documented first date of attendance was 10/11/2021. Child #2 and Child #3's documented first date of attendance was 08/30/2021.
4. The provider confirmed physicals had not been received for within 30 days of the first day of attendance for Child #1, #2 and #3.Plan of Correction: Physicals will be requested from the parent to provide documentation as soon as possible.
