Comments:
A monitoring inspection was conducted on 04/28/2022. There were 12 children present, ranging in ages from 4 months to 2 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-920
Description: Based on documentation review and interview with the provider, the menu for the current one-week period did not list any substituted foods.
Evidence:
1) On the date of inspection, the provider stated that the lunch that would be served to the children was peanut butter and jelly sandwich, green beans, apples and milk. However, the menu for the day listed lunch as egg, macaroni noodles, corn, peaches and milk. No substitutions were listed on the menu.

Plan of Correction: I will make sure to list substituted food. I corrected the substituted foods on the list.

Standard #: 8VAC20-800-50-A
Description: Based on documentation review, the family day home did not keep and accurate written record of children in attendance each day.
Evidence:
1) On the date of inspection, the inspector observed twelve children in attendance, however, the attendance record only had eleven children documented in attendance for the day.

Plan of Correction: I will make sure to document the daily attendance for all the children. I corrected the attendance record.

Standard #: 8VAC20-800-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) On the date of inspection, the record for Child #1 did not contain the address of one designated person to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: I contacted parents and updated the address of one designated person.

Standard #: 8VAC20-800-70-A
Description: Based on record review, parents were not provided in writing all required information before the child's first day of attendance.
Evidence:
1) The record for child #2 did not contain signed acknowledgement of the parents having received the fdh's policies for the administration of medications.

Plan of Correction: Got the signed acknowledgement of the parents having received the fdh's policies of the administration of medication.

Standard #: 8VAC20-800-210-A
Description: Based on record and documentation review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1) The record for Staff #1 did not have documentation of having completed any training hours in the year 2021.
2) The record for Staff #2 had 13.5 training hours documented for the year 2021.
3) The record for Staff #3 had 9.5 training hours documented for the year 2021.

Plan of Correction: All the caregivers will obtain the necessary hours of training within two months.

Standard #: 8VAC20-800-570-A
Description: Based on observation, the provider did not ensure ensure that each caregiver does not exceed 16 points to provide direct care and supervision of each child.
Evidence;
1) On the date of inspection, 12 children were in attendance for a total of 37 points. Only two caregivers were present on this date, putting them over by one point of the child/caregiver points for supervision.

Plan of Correction: I will make sure to have correct number of caregivers needed for the supervision of children.
