The vendor shall maintain, in a way that is accessible to all caregivers, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential.

Comments:
An unannounced subsidy health and safety inspection was conducted on 05/05/2022 from 10:40 am - 1:10 pm. At the time of entrance there were ten children in care with two caregivers. The ages of the children counted for a total of 27 points.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-180-B
Description: Based on review, the vendor failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. The attendance sheet/parent sign in record lacked documentation of the arrival of three of ten children in care.

Plan of Correction: It's done correct.

Standard #: 8VAC20-790-250-B
Description: Based on record review and interview, the vendor failed to ensure that within the first 90 days of employment or service all caregivers shall complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Education.

Based on record review and interview, the vendor failed to ensure that caregivers who work directly with children shall annually complete the department's health and safety update course.

Evidence:
1. Caregiver 2, hire date 12/12/2017, has not completed the 10-hour Virginia Preservice Training.
2. Caregiver 2 has not completed the 3-hour annual Virginia Health and Safety updated training for child care staff.
3. The provider confirmed that caregiver 2 had not completed any of the Virginia Preservice training.

Plan of Correction: Training will start for assistant.
