Technical Assistance:
Standard 610 - Use of media such as, but not limited to , television, videos, video games, and computers shall be limited to not more than a total of two hours per day.

Check back yard daily for standing water inside of outdoor toys and equipment.

Keep an eye on the cracks on the seats of the 4-way spring rocker.

Reminder: Two shelter-in-place and one lock down practice drills are required annually.

Standard 520 - Infants shall be provided with a crib for napping/sleeping.

Discussed activities for infants.

Comments:
An unannounced renewal inspection was conducted on 05/05/2022 from 10:40 am - 1:10 pm. At the time of entrance there were 10 children in care with two caregivers. The ages of the children counted for a total of 27 points. Children were observed watching an educational show on television, eating lunch and napping. Records were reviewed for two caregivers and ten children.

Violations were found in the areas of administration, caregiver training, physical equipment and environment, care of children, emergencies, nutrition and carbon monoxide detector requirements.

These violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22.1-289.058
Description: Based on interview, the provider failed to ensure that the home shall be equipped with at least one carbon monoxide detector.

Evidence:
The provider stated that a carbon monoxide detector had not been installed in the family day home.

Plan of Correction: Will get a carbon monoxide detector.

Standard #: 8VAC20-800-790
Description: Based on observation, the provider failed to ensure that the following telephone numbers shall be posted in a visible area close to the telephone:
a. A 911 or local dial number for police, fire and emergency medical responders;
b. The responsible person for emergency backup care;
c. The regional poison control center.

Evidence:
1. Emergency phone numbers for 911, poison control and a responsible person for emergency backup were not posted in a visible area close to the telephone.
2. The provider confirmed that these phone numbers were not posted.

Plan of Correction: Telephone numbers will be posted by phone.

Description: Based on observation, the provider failed to ensure that children shall not be allowed to eat or drink while lying down.

Evidence:
1. Child 2 was given a transition sippy cup of milk to drink, at approximately 11:20 am. Child 2 proceeded to drink the sippy cup of milk while lying down in the pack and play.

Plan of Correction: Children will not drink while laying down.

Description: Based on record review, the provider failed to ensure that assistants' records shall contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Evidence:
1. The assistant lacked documentation of a second reference check.
a. Caregiver 2 has a hire date of 12/12/2017.
2. The provider confirmed that a second reference check was not on file.

Plan of Correction: Assistant will have another reference on file.

Description: Based on record review, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1. The most recent immunizations for child 1, age 17 months, were conducted on 06/02/2021.

Standard #: 8VAC20-800-210-A
Description: Based on interview and record review, the provider failed to ensure that in addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
1. The provider stated that no training classes had been obtained for caregiver 1 and caregiver 2 in 2021 or 2022.

Plan of Correction: Provider and assistant will do the training.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider failed to ensure that outside areas of the family day home shall be maintained in a safe condition.

Evidence:
1. One wooden fence picket on the playground gate has come away from the post. Two nail points are exposed at the bottom.
2. Four areas of cracked plastic were observed on the "Range Rover" play car located in the backyard play area. These areas of cracked plastic are sharp.

Plan of Correction: It will be taken care of ASAP.

Description: Based on observation, the provider failed to ensure that Windows used for ventilation shall be securely screened.

Evidence:
1. The screens for the two open windows in each child care room did not securely fit inside the window as they were open at the bottom.

Plan of Correction: Window screen will be fixed.

Standard #: 8VAC20-800-600-A
Description: Based on observation, the provider failed to ensure that Infants and toddlers shall be provided with opportunities to:
*Play with a wide variety of safe, age-appropriate toys;
*Reach, grasp, pull up, creep, crawl, and walk to develop motor skills.

Evidence:
1. Child 2, age 12 months, and child 3, age 9 months, were not provided with any toys to play with or any opportunity to reach, grasp, pull-up, creep or crawl from 10:40 am - 1:10 pm.
a. Child 2 was placed in a pack and play for a total of approximately 90 minutes without any activity. There were no toys provided for child 1.
b. Child 3 was placed in an infant seat for a total of approximately 2 hours and 20 minutes without any activity or toys to engage with.
2. Both children were awake during this time.

Plan of Correction: Children will be given toys to play with.

Standard #: 8VAC20-800-600-B
Description: Based on observation, the provider failed to ensure that infants and toddlers shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. The intervening time period between confinements shall be at least one hour.

Evidence:
1. Child 2, age 12 months, was observed placed inside of a mesh sided pack and play from 10:40 am - 11:05 am when she was taken out by caregiver 2.
a. Child 2 was placed back in the pack and play at approximately 11:16 am until approximately 12:20 pm.
2. Child 3, age 9 months, was observed placed in an infant seat from 10:40 am until approximately 11:25 am when he was taken out for approximately 10 minutes.
a. Child 3 was placed back in the infant seat at approximately 11:35 am where he remained until the inspector left at 1:10 pm.

Plan of Correction: Children will be moved every 30 minutes.
