Comments:

An unannounced hybrid monitoring inspection was conducted on 4/27/22. The inspection was not closed on site. There were 4 children (14 points) in direct care of 2 caregivers. The children were observed sleeping, waking up from nap and going outside for a walk. There were no medications on site. 2 caregivers, 2 children's records, emergency drill records and 1 family pet's record was reviewed on site. The playground used by the children was observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-800-800-B
Description:	Based on record review, the provider did not review the emergency plan at least annually. Evidence: The emergency preparedness plan was last reviewed on 1/12/21. The plan was not reviewed since that date.
Plan of Correction:	We have reviewed the emergency preparedness plan since the inspection.
Standard #:	8VAC20-800-210-A
Description:	Based on staff interview, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Evidence: The provider stated that Caregiver # 1 and the provider did not complete 16 hours of annual training as required by the standards.
Plan of Correction:	We will use the information and links the inspector gave the provider. We will obtain the hours necessary.
Standard #:	8VAC20-800-460-D
Description:	Based on records review, the provider did not maintain current rabies vaccinations for pets at home. Evidence: 1. The most recent rabies vaccination for the provider's dog expired on 4/7/22. 2. The rabies vaccination record for the provider's pet cat were not available for review.
Plan of Correction:	The record for the cat will be sent soon via email. We are going to schedule an appointment for the dog to get his rabies vaccination.
Standard #:	8VAC20-800-510-A
Description:	Based on observation and staff interview, an infant was not provided with a crib while napping. Evidence: 1. At the beginning of the inspection, at approximately 10:50 am, Child # 1 was observed to be sleeping in her car seat. The car seat did not have it's base, which helps to align the car seat. 2. The provider stated that Child # 1 was asleep upon arrival, and she did not want to wake her by moving her to the crib. 3. A crib was available to use in the infant's nap room.
Plan of Correction:	The infant was moved to a crib during the inspection.
