Comments:
An unannounced renewal inspection was conducted on April 19, 2022 between 10:30 am and 11:15am. There were 9 children in the direct care of 2 staff members. Staffing records, child records, and the physical space were inspected and reviewed. Children were participating in Montessori work, washing hands, and eating snack. Description: Based on record review, the provider did not ensure repeat background searches every 5 years from date of previous search.

Evidence: Household member #1 central registry search expired August 25 2021.
Staff Member #1 Central registry search expired on August 25 20, 2021

Plan of Correction: Will submit background checks for staff and household member.
