A renewal inspection was initiated and concluded on 4/29/2022. There were 8 children present, with the provider and one assistant caregiver supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, caregiver training, physical equipment and environment, care of children, preventing the spread of disease, emergencies, and nutrition. A total of 3 child records and 5 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-60-B Description: Based on record review, 1 of 3 children's records did not contain all required information.
Evidence:
1. The record for Child #2 did not contain the address of the mother's place of employment.Plan of Correction: I will get the information from the parent.
Standard #: 8VAC20-800-70-B Description: Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of information required.
Evidence:
1. The Information for Parents document in the record for Child #1 was not signed by the parent.Plan of Correction: I will have the parents sign.
Standard #: 8VAC20-800-90-A Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized before the child's first day.
Evidence:
1. Child #1 began attending the family day home on 3/7/2022.
2. The immunizations in the record for Child #1 were dated 4/5/2022.Plan of Correction: The immunizations were obtained and placed in the child's record.
